MUST BE ICD10 CERTIFIED TO BE CONSIDERED Job Description/Requirements:? As a Plan of Care Coordinator you will:
Demonstrate Appropriate Clinical Knowledge and Skills.
Demonstrate knowledge of medical terms, medical abbreviations, medical charts, medications, and medical protocols.
Demonstrate knowledge of ICD-10 CM Coding standards.
Possess the ability to accurately review an OASIS assessment and identify inconsistencies in OASIS assessment items in accordance with OASIS guidelines.
Demonstrate knowledge of the Medicare definition of Homebound status.
Demonstrate knowledge of all documents and resources required by policy and procedures for accurate coding.
Demonstrate knowledge and Accurate Interpretation of State and Federal Regulations.
Follow HIPAA requirements 100% of the time (e.g. never discloses confidential materials and/or information or makes it accessible to unauthorized personnel.)
Possess the ability to assist staff with interpretation of regulations.
Demonstrate knowledge of proper procedure for reporting regulation violations.
Demonstrate Ability to Communicate Effectively, in both oral and written forms.
Maintain accurate and professional communications.
Possess the ability to submit adequate documentation when requested.
Possess the ability to give direct and understandable response to questions.
Articulate in a wide range of communication situations.
Possess the ability to communicate in a positive manner.
Demonstrate proper phone etiquette in accordance with company policy
Possess the ability to assists with emergency calls or situations when needed.

Requirements of Qualified Candidates:?
Currently licensed as a registered nurse in the state(s) of agency operation
Minimum of five years? experience as an RN
Minimum of two years? experience in home care
MUST BE ICD10 CERTIFIED TO BE CONSIDERED
Minimum of one year of coding experience?
Minimum of one year of OASIS experience (Certification in OASIS preferred at hire, required within 90 days)
Available at all times during operating hours
Able to read, write and comprehend English
Strong time management, prioritization and organizational skills
Ability to work in a fast-paced, multi-task environment?
RN certified in ICD 10 CM and OASIS to review SOC and ensure accuracy. RN will work from home and will be required to complete the review, code accurately, and make recommendations on the scoring for 18-22 cases daily ?
